I have heard that the effect of a botox surgery to clear the frown lines is not permanent. Is there a permanent surgery alternative for this?

The body changes with age, so nothing can be termed as permanent. If it were permanent, it would be something on the lines of "eternal youthfulness", which is still a dream as of now.

No, the result is not permanent. The patients have to come back and take the Botox every 6 months to maintain the result.
